Bagged Salad Mix Recalled After Dead Bat Found in Bag

Walmart is recalling their Marketside Spring Mix Salad after the supplier "Fresh Express was notified that extraneous animal matter was allegedly found in a single container of the salad".   That extraneous matter was a dead bat.

While of course, everyone worries about rabies, but this would be an extremely low risk.  Rabies virus is an enveloped virus and does not survive in the environment well if at all. While there have been some rare, laboratory-created instances of it be transmitted through aerosol with depleted oxygen, this is really a non-factor.  The bigger risks are the normal foodborne pathogens that can be carried by bats, including Campylobacter, Listeria, Salmonella, Shigella, and Yersinia as well as some more exotic bacterial types.  Depending on where the bat entered the system would determine the amount of dissemination of these types of organisms.

Now how does bat get into a bag?  If not intentionally placed there by someone, and one had to guess....it would most likely come into the packaging area of the processing plant at night.  The greens are chopped and washed, so unlikely that it came in the loads of produce (unless some component was not chopped).  More likely, this flew into the plant at night when doors were left opened and took refuge in the equipment.  If this were the case, it occurred after sanitation.  When the greens started flowing, the little dude was crushed and loaded into the bag.  All a guess of course, but if true, would probably be in one of the bags from early in the production run..  Prevention....keeping doors shut at night, especially dock doors. X-ray equipment would have likely found this if in place.

Packaging Error Results in Allergen Recall of Nutrition Bar

A nutrition bar company is recalling their crunchy Slim Peanut Butter bars after it was discovered that a product with milk allergen was accidentally mislabeled with the Slim Peanut Butter packaging that does not list milk allergen.

In applying Preventive Controls in the operation that made this product, having a properly implemented Allergen Preventive Control at the point of labeling would help prevent this.  This would include having a step to check the labels for a match to the formulation and then having verification (another person checking, a record review of the labeling record, direct observation of the label check, etc).

This mistake occurred during one shift, but once the depth of the recall expands to store level, this recall can become fairly costly.

OK Firm Recalls Nearly 1 Million Pounds of Breaded Chicken for Metal Complaints - Linked to Conveyor

A OK establishment is recalling close to one million pounds of breaded chicken products for the potential of metal contamination.  According to the notice: "The problem was discovered on March 21, 2017 after OK Foods Inc. received five consumer complaints stating that metal objects were found in the ready-to-eat chicken products and by FSIS inspection personnel during verification activities. After an internal investigation, the firm identified the affected product and determined that the objects in all the complaints came from metal conveyor belting." 

It would be interesting to know if the firm had a metal detector in place.  With a breaded chicken product, one would expect to see one.

NY Creamery Expands Recall, Product Linked to Illnesses. 2 Deaths

A NY creamery expanded its recall, posted on 3/7/16, as the CDC has linked the product to 6 cases of listeriosis.  There have been 2 deaths.

The product had national distribution, and with the window of time until the product was recalled, there may be additional cases that may be reported.

Listeria can be problematic in soft cheeses in that the product will support the growth of Listeria  We have seen a number of cases of listeria related to soft cheeses, so it is incumbent on those packing soft cheeses to have an aggressive Listeria control program. 
Harder cheese have a lower water activity and will not support the growth of Listeria.  While the risk is lower, the food plant-environmental pathogen still needs to be controlled.

Cheese Ravioli Product Recalled for Missing Allergen in "Contains" Statement

Gerber is recalling its Cheese Ravioli Pasta Pick-ups for a allergen labeling issue - while allergen egg is listed in the ingredient listing, it was missed in the 'Contains' statement.

FALCPA (Food Allergen Labeling and Consumer Protection Act of 2004) requires that the label of a food that is fabricated from two or more ingredients declare each ingredient by its common or usual name.  It can do this either in the ingredient listing or it can highlight the allergens included in the product with a "Contains" statement.   However, FDA provided further guidance that if the company chooses to use the "Contains" statement, all major allergens in the product must be listed.  (FDA referenced documents below).

So in this case, while the ingredient  egg was in the ingredient listing, it was missed in the "Contains' statement.

NY Creamery is Recalling Soft Cheese Products After Positive Listeria Tests

UPDATE - 3/8/17  Whole food recalls cheese supplied by Vulto Creamery.

A NY company is recalling its soft raw milk cheeses after FDA and NY Department of Ag and Markets found that product may be possibly contaminated with Listeria monocytogenes.

MA Establishment Recalls Shaved Beef for Plastic Foreign Material

A Massachusetts establishment is recalling shaved beef after a retail outlet notified the firm that the product had "readily visible, long, thin strands of clear soft plastic from packaging material".  They further stated that because "the plastic is readily visible and identifiable, it is highly unlikely that it would be consumed and, therefore, the health risk is extremely low."

Ready-made Salads Recalled Due to Potential Listeria Contamination of Cheese Ingredient


Ready Pac's three production facilities (CA, GA, NJ) are recalling one type of salad due to the potential for Listeria in the Pepper Jack Cheese Ingredient that was used in the salad.  The company stated the recall was conducted after they were notified by their cheese supplier of the issue.  The name of the cheese company was not made available in the release.

Tortilla Products Recalled After Complaints of Brittle Plastic Contaminaiton

A Texas establishment is recalling ~23,500 lbs of beef & cheese tortilla products after the company received complaints of foreign objects in the product, specifically clear, brittle plastic.  There have been no reported injuries.

FDA Advisory Against Eating Cheese from PA Goat Cheese Operation

FDA is advising consumers not to eat cheese from a PA firm due to Listeria.  The issue was discovered when "Apple Tree recalled four lots of products manufactured in March and July 2016 on September 20, 2016, after samples of these lots collected by the Pennsylvania Department of Agriculture (PDA) tested positive for Listeria monocytogenes". FDA conducted an inspection, and in addition to finding unsanitary conditions, they found Listeria in the environment and the finished product."

Apple Tree Goat Dairy is a family run operation that focuses on boutique / small lot production of organic cheeses.  And while it is easy to get caught up in the mystique of this type of operation, issues can occur.  Just like the Chipotle case, firms dealing with food still must execute on the fundamentals. That includes understanding the risks, putting in controls for those risks, monitoring those controls, and verifying that they work.  This is all part of the craft..it is not just making products, but making products in a safe and sanitary fashion.  And for a cheese operation, that means controlling Listeria.

We see this all too often with food entrepreneurs...they have a great idea on making a product, but never make sufficient effort to learn the fundamentals of food safety and sanitation.  The microorganisms don't care if is organic, family run operation or even a multi-billion dollar facility...if these organisms are not controlled, they will infiltrate and establish themselves in the facility and potentially the product.

Pancake Mix Recalled Due to Use of Potentially Contaminated Milk Powder Ingredient

Two companies are recalling pancake mix that used the recalled milk powder as an ingredient.  That milk powder had been recalled a week ago when FDA had seized product due after finding Salmonella in the facility but not the product.

This is interesting in that although the ingredient has never tested positive and there have been no related illnesses, downstream processors are recalling product for their supplier's less-than-clean facility.  Granted that facility did have a persistent strain of Salmonella found throughout the environment, but without knowing the particulars of the facility, it is hard to evaluate the true risk.  But there is a risk, although small, that if Salmonella were present and the consumer making pancakes spills the mix on the counter.  And of course, there are those who may sample the raw batter.

Cooked Meat Recall Expanded for OK Firm Due to Additional Complaint of Undercooked Meat

An OK company drastically expanded it Nov. 26, 2016 recall for potentially undercooked ready-to-eat chicken from ~20,000 lbs to ~2,000,000 lbs.  The recall now includes about 3 months of production (original was 2 day lots) and covers a wider variety of product.   The expanded recall comes because of an additional complaint of raw product on November 28th.

No illnesses have been reported.

Sabra Hummus Recall Leads to Secondary Recalls

Greencore and Sky Chefs are recalling prepared sandwiches that were made with the recalled Sabra hummus.  Sandwiches were delivered to 7-Eleven stores in various markets including Pittsburgh, PA.

Taylor Farms is recalling various veggie trays and boxes that contain the Sabra humus.

Oreo Fudge Creames Recalled Due to Milk Allergen, Even Though Listed in Precautionary Statement.

A specific type of Oreo cookie is being recalled after someone had an allergic reaction to milk, and milk was not declared on the label.  It was however, listed in the precautionary statement - "Manufactured on equipment that processes milk.."

The precautionary label is not part of any official allergen labeling rule, so it doesn't provide protection.  Basically you hope someone with a given allergy such as milk would see that and then avoid eating it.  Didn't happen.

From FARRP  - "FDA specifies that such [precautionary] labeling cannot be used as a substitute for good manufacturing practices such as allergen identification and control, cleaning of shared equipment, and segregation during processing. Some consumers believe manufacturers use precautionary labels as a way to protect themselves from legal action in case a food causes a reaction. This is not typically the case since this type of label should only be used by food manufacturers when there actually is a possibility (however small) that the food may contain an allergen which is not an ingredient but may be present as a result of the manufacturing process."

Venture Capitalist Start-Up, Soylent, Recalls Meal Replacement Due to Illnesses

A venture capitalist start-up nutrition company, Soylent, is recalling their nutrition snack bar after complaints of vomiting, diarrhea, and nausea.

First, who is buying this stuff?  One look at the website and it looks like something a bunch of computer hipsters came up with.  With all these real food companies with flat or decling sales, I am not sure why people feel that a bunch of techno-geeks can make a better product.

Limited Amount of Lunchables Recalled Due to Allergen Mislabeling

Kraft Heinz is recalling a limited amount of Lunchables product due to a mismatched label that resulted in allergens not being declared.  According to the release  "Lunchables Ham and American Cracker Stackers” products were incorrectly labeled with the back label for a “Nacho Lunchable” product. The back label contains the product ingredient statements and as such, the “Lunchables Ham and American Cracker Stackers” products that were mislabeled do not declare wheat and soy on the label.

Onion Rings Recalled Due to Allergen Mislabeling - Reason for Implementing Preventive Controls

McCain Foods issued a recall notice for Onion Rings and then reissued the notice to expand the recall to additional labels.  The pre-labeled packaging did not list milk on the label although it was included in the formulation.

This is the type of recall that the Preventive Controls rule was designed to help prevent.  By having a formalized check of the labels at the receipt of those labels and then again at the time of packaging would have provided 2 opportunities to compare the label to the formulation.  Of course, it is important to have the sub-ingredients listed, in this case, they are probably purchasing the breader / batter that will be used to coat the onion rings.

Instituting such a check is not overly difficult. It just takes a few minutes to compare the label to the formula.   However, the cost of recall resulting from non-compliance can run into the millions of dollars.

Chicken Nuggets Recalled Due to Plastic Pieces

Tyson is recalling 132, 520 pounds of cooked chicken nuggets for plastic pieces.  Product was packed in an institutional 20lb size that was shipped to Pennsylvania and the 5lb retail pack was shipped nationally.

The recall was initiated after consumer complaints were made.  According to the report "the plastic material ranged in size from 21mm in length and 6.5mm in diameter and may have come from a round, hard plastic rod used to connect a plastic transfer belt. The firm said the products pass through a metal detector, but the plastic is not detectable to this technology".
In May, Foster Farms recalled chicken nuggets due to foreign material.  And in March, Purdue recalled chicken nuggets due to plastic pieces.  In 2014, Tyson recalled chicken nuggets due to plastic pieces.  It may make sense for those making chicken nuggets to invest in X-ray technology.

Blue Bell Recalls Two Ice Cream Products due to Potential Listeria Contamination

Blue Bell Ice Cream is recalling two flavors of product for potential Listeria contamination.  There have been no illnesses reported.  According to the company's press release, product was shipped to 10 states, primarily in the Southeast US.

Last year, Blue Bell shut down its 3 plants after their ice cream product was linked to a Listeria outbreak where 10 people were infected.  The company reopened the plants with increased controls and under increased government scrutiny.  In January of 2016, the company issued a release that their controls were effective, although they were still finding Listeria in the environment.

In this particular case, it looks as though the supplier of the cookie dough, Aspen Hills, may be at fault.  Blue Bell discovered the issue through testing of incoming products and notified Aspen Hills who then issued a recall.  This recall notice is posted on the Blue Bell website.  Unfortunately, product was released before these test results came to light.  Although Blue Bell states they do test and hold (product is not released before results come back), that test and hold looks to have been focused on finished product testing (and perhaps not ingredient testing?).

Of course this cookie dough is not technically raw.  It is product that is made to resemble raw cookie dough, but is a ready-to-eat product.  Being that it is ready-to-eat, it should be free of pathogens such as Listeria.

Goat Cheese Recalled Due to Listeria Positive Sample

 A small Pennsylvania goat cheese manufacturer is recalling goal cheese products after PA Dept of Ag tested and found Listeria in the product.   No illnesses have been reported.
