The Botulism Outbreak(s) That Provided Impetus For Standardized Canning Procedures

An article in the Smithsonian magazine discusses the impact of a botulism outbreak in 1919 / 1920 that lead to the development of standardized procedures for canning.  These standardized procedures for processing and inspection were not initially federal regulation, but were a state regulation in California and an industry standard used by the National Canners Association.  Must of this was the basis for the federal regulation that came in the 1970's - the Low Acid Canned Food Regulation, which were also enacted after a series of issues related to canned salmon.

The 1919 / 1920 outbreak were there were close to 20 deaths had been associated with canned olives.  But there had been other cases throughout the first half of the twentieth century.  This link to the Califormia Dept of Health website on the the topic.
"From 1899 to 1949, there were 483 outbreaks of botulism reported in North America (the United States and Canada) involving 1,319 case patients and 851 deaths. Of these outbreaks, cases and deaths, 184 outbreaks, 416 cases, and 252 deaths were in California. Additionally, 40 deaths occurred in other states from products prepared in California. In 1919 alone, olives commercially canned in California caused 17 deaths (7 in Ohio, 5 in Montana and 7 in Michigan)."
"All of this was aggravated by sensational newspaper publicity. As a result, botulism became a reportable disease in November 1920. Also, on August 7, 1920, the State of California Board of Health (Board) adopted the first regulation of olive production requiring sanitation throughout the canning facility and a mandated thermal process (specified time and temperature in heating canned foods to render then commercially sterile). In 1920-21 some 20 deaths occurred from botulism linked to canned spinach produced in California. As a result, in April 1921, the Board adopted a resolution covering canning and sterilization of spinach specifying definite fill-in weights, cut-out weights, initial temperatures, and sterilization processes."
This is a good reminder for the restrictions on canning of low acid foods for commercial sale.

Nacho Cheese Sauce and Botulism - Case Findings and Recommendations

In May of 2017, there was a botulism outbreak involving nacho cheese sauce served at a California gas station.  It is easy to see from this that the gas station did not monitor temperature, so likely that the units were possibly shut off.  This is a TCS food item, and with that, strict temperature control is required.   Unfortunately, this type of nacho cheese sauce if often treated as a shelf-stable item.   In the recommendations below, it states that the cheese can be hot held for up to 6 days....so much for me eating nacho cheese on anything served on a food bar...not that I have eaten it ever in the past.

CDC Report - National Botulism Surveillance Summary 2015

CDC released the summary of botulism cases for 2015.  Botulism is the disease caused by the toxin produced by Clostridium botulinum.  The toxin is a neurotoxin that stops neurotransmission resulting in paralysis, or loss in muscle function.  Botulism poisoning can result in death if not treated.  In most cases, it will result in hospitalization with the patient on a breathing apparatus due to loss in the ability to breath (paralysis of diaphragm).

The type we are most familiar is foodborne botulism where the organism grows in the food producing toxin.  But there is also infant botulism where the organism becomes part of the intestinal microflora of an infant.  A similar issue can happen in adults when their intestine is subject to colonization due to illness associated with antibiotic administration and decline of normal intestinal microflora.  Wound botulism occurs when the organism gets into a would and begins to grow.

The largest amount of cases reported in 2015 were infant botulism (141).  No deaths were reported.

There were 39 confirmed cases of foodborne botulism.  The one outbreak occurred when improperly canned potatoes were used to make potato salad at a church potluck dinner.  This resulted in 27 cases.
Another outbreak was associated with fermented seal flipper (4 cases), and then another was associated with beets roasted in aluminum foil and kept at room temperature for several days then made into a soup (2 cases).

One Man Dead, A Mom Still Hospitalized after Three Weeks, Both Consumed Contaminated Nacho Cheese Sauce

One woman's story, associated with the botulism in gas station nacho sauce case, highlights the issues for someone with botulism poisoning.  A few hours after eating Doritos drizzled with nacho cheese sauce, she felt fatigue and by the next day, she had blurred vision followed by vomiting and difficulty in breathing.  These symptoms are caused by the  neurotoxin produced by Clostridium botulinum as it grew in the nacho cheese sauce. Once the toxin contaminated food is consumed, "Botulinum toxin causes flaccid paralysis by blocking motor nerve terminals at the neuromuscular junction. The flaccid paralysis progresses symmetrically downward, usually starting with the eyes and face, to the throat, chest, and extremities. When the diaphragm and chest muscles become fully involved, respiration is inhibited and, without intervention, death from asphyxia results". (FDA Bad Bug Book)

There has also been one death being associated with the tainted nacho cheese.  A man had been on life support.  This has not yet been confirmed by State officials.

There are now 10 people that have been affected from eating at this establishment.

Deer Antler Tea Source of Botulinum Illnesses

A deer antler tea product may be responsible for two cases of botulism in Los Angeles.  Botulism is deadly disease caused by the toxin produced by Clostridium botulinum.   This neurotoxin stops neural signals resulting in double vision, slurred speech and difficulty swallowing.  If not treated, it can lead to death by suffocation.  
 How did the botulinum toxin get into this antler tea product? If one had to make a guess, it was after the extraction process.  To make the antler tea, antlers would be heated to pull out the marrow creating a broth and then this would be cooled to make a gelatinous material that would be packaged.  As in a typical C. botulinum growth scenario, when the product is heated to create the marrow broth, the high temperatures kills the competing spoilage organisms and creates an anaerobic environment in the marrow broth.  When the product is cooled, but held above refrigeration temperatures, C. botulinum grows and produces its toxin.  Generally an item like this would either need to be refrigerated or acidified.  A preservative can also be added to prevent growth.
The benefits of drinking antler tea are said to include helping with arthritis, preventing osteoporosis, promoting the health of the joints and repairing connective tissues, nourishing the spleen and the kidney, a defense against cancer cells, boosting the HB count in the blood, treating sexual disorders, increasing stamina and actual physical strength while also improving mental capacity, helping women who have vaginal discharge problems, flooding and spotting and it also helps in stanching excessive uterine bleeding.  Who knew.

Improperly Canned Potatoes Responsible for Botulism Outbreak in April, 2015

In April of  2015, a botulism outbreak occurred that was associated with food served at a Church Potluck Meal.  In this outbreak, 29 reported becoming ill.  This was the largest botulism outbreak in the last 40 years.

The source was home canned potatoes.  The potatoes were canned in hot water bath and not a pressure canner.  From the CDC:
The attendee who prepared the potato salad with home-canned potatoes reported using a boiling water canner, which does not kill C. botulinum spores, rather than a pressure canner, which does eliminate spores (2). In addition, the potatoes were not heated after removal from the can, a step that can inactivate botulinum toxin. The combined evidence implicated potato salad prepared with improperly home-canned potatoes, a known vehicle for botulism (3).
Unfortunately, we hear of too many who still use a hot water bath to can low acid foods.  Outbreaks like this remind us of the importance of following scientifically developed canning recipes, especially the use of a pressure canner for low acid foods.

On April 21, 2015, the Fairfield Medical Center (FMC) and Fairfield Department of Health contacted the Ohio Department of Health (ODH) about a patient suspected of having botulism in Fairfield County, Ohio. Botulism is a severe, potentially fatal neuroparalytic illness.* A single case is a public health emergency, because it can signal an outbreak (1). Within 2 hours of health department notification, four more patients with similar clinical features arrived at FMC's emergency department. Later that afternoon, one patient died of respiratory failure shortly after arriving at the emergency department. All affected persons had eaten at the same widely attended church potluck meal on April 19. CDC's Strategic National Stockpile sent 50 doses of botulinum antitoxin to Ohio. FMC, the Fairfield Department of Health, ODH, and CDC rapidly responded to confirm the diagnosis, identify and treat additional patients, and determine the source.

Food Safety Quick Hits - Hyperlinking the News for May 29th, 2015

An Ohio firm is recalling jars of salsa after discovering jars were not properly sealed. The titles of the various news articles call it a potential botulism concern, unsealed jars are more likely to be a spoilage issue.

The FDA issued a recall notice for a Colorado company that is recalling raw Macadamia nuts due to the presence of Salmonella.  The Salmonella was found by FDA through routine product testing.  No illnesses have been reported.

A California company is recalling candy - coconut chews because of the potential to contain small stainless steel wire.  It looks to be that a screen within the manufacturing process is the source.  Perhaps a screen broke or was fraying?

In California, patrons filed a lawsuit against a Mexican restaurant that reportedly made them ill.   The LA County Health Department investigated after seeing a number of complaints on Yelp, the social media site for restaurant reviews.

The CDC is investigating a multistate outbreak of Salmonella paratyphi B that may be linked to raw tuna used for sushi.  The epicenter for the cases is in the Southeast US.  As of May 21, 2015, a total of 53 people from 9 states have reported being ill.    CDC is still investigating the source.
Interesting, this variant does not cause parahyphoid fever, which would be characterized by high fever, but is more similar to the symptoms of normal Salmonella  which includes diarrhea, fever, and abdominal cramps 12-72 hours after being exposed.

In Wisconsin, 60 people report becoming ill from a Salmonella infection after eating pork carnitas served by a local market.  According to Wikipedia, "Carnitas, literally "little meats," is a dish of Mexican cuisine originating from the state of Michoacán. Carnitas are made by braising or simmering pork in oil or preferably lard until tender. The process takes three or four hours and the result is very tender and juicy meat, which is then typically served with chopped coriander leaves (cilantro) and diced onion, salsa, guacamole, tortillas, and refried beans (frijoles refritos)."  Sounds delicious except for the Salmonella part.

Finally, some reasons why not to take a cruise.  In a report issued by some group that supports journalism in the public interest, they call attention to all the bad stuff that can happen on a cruise ship.  From the report: "Last year over 1,700 passengers and crew members fell sick from gastrointestinal illnesses like norovirus. Since 2012 at least seven children have drowned or nearly drowned in cruise ship pools that rarely have full-time lifeguards. This year, a 21-year-old college student fell overboard and was never found -- one of at least two dozen incidents in the last two years in which cruise passengers or crew have gone overboard, according to media reports."

Woman Contracts Botulism Poisoning from Improperly Canned Carrots

A North Carolinian woman contracted botulism poisoning after almost eating an improperly processed carrot.   Instead of pressure canning the jars of carrots, she used the hot water bath canning technique (which is only appropriate for high acid foods).  After putting the carrot in her mouth, she realized it was 'off' and spit it out.  However, she ingested enough toxin to get the illness with just putting that carrot in her mouth.

Unfortunately, too many people still process jars of low acid foods (carrots, peas, beans) in boiling water rather than using the pressure canner.  No doubt this woman would have fought against using the pressure canner..its too dangerous, the carrots are not firm, etc.  After 11 weeks in the hospital, maybe she can be a warning to others.

 An Ashe County woman learned the hard way her food-preservation techniques were wrong.

It only took one bite.

Five days later, an Ashe County woman lay in the hospital, on a ventilator, unable to breathe.

The woman, who’s name has not been released, told health officials she didn’t even swallow the carrot. She opened the home-canned jar of carrots, tasted one, decided it looked and tasted off, and spit it out.

But that was enough to give her botulism, sending her to the hospital for an 11-week stay.

Botulism Outbreak in Ohio Linked to Home Canned Potatoes, Confirmed Cases Now 21

The source of the botulism outbreak at the Ohio church potluck is most likely the home canned potatoes used to make the potato salad.  In this outbreak, the number of confirmed cases of botulism poisoning is now 21.

While potatoes can be safely canned (if done according to the directions provided by the National Center for Home Food Preservation), it is important to use the right potatoes, the right size, and the use of a pressure canner.  While there has been no further information provided at this point related to the canning procedures used or issues seen on other containers processed that may be still in storage, potatoes can become an issue if not done correctly.  First, potatoes are low acid, so a pressure canner must be used.  Second, it is important the type and the size are correct to allow proper heat movement within the jar (convection heating).  If the size is too big or the potatoes break down during processing, the heating parameters within the jar will change, thus affecting the ability of heat to reach the cold spot of the jar. 

Another issue is the use of home canned foods for volunteer events.   Generally,  bringing home canned foods to volunteer events is discouraged.  In Penn State's Volunteer Food Safety Curriculum, Cooking for Crowds, it states in Chapter 4:
As a precaution, never accept any home-canned or home-preserved food [for volunteer events]. Life-threatening foodborne illness can occur from food that has been preserved incorrectly
 The same must be said for using canned foods as an ingredient in dishes that will be brought and served at group functions.  There are no regulations for these volunteer groups and their functions however and so it comes down to the individual groups to require training and to institute policies.

Getting home preservers to follow scientifically developed recipes is not an easy.   Cases like this are reminders that improper canning procedures can lead to deadly consequences.

Over 20 Cases of Botulism with one death from Food at Church Potluck

One death and 23cases of botulism are being reported after the individuals ate at a church potluck in Lancaster. Ohio.

Antitoxin has been administered to the victims, which can reduce severity of the illness. 

The food has not yet been identified.

Botulism is the disease caused when the toxin produced by Clostridium botulinum is ingested.   The toxin is produced by the organism as it grows in a food. Generally we do not see many cases of this deadly disease, but it can be a risk when certain foods are not properly prepared. 
  • Improperly processed low acid canned foods like green beans that are not properly pressure canned and then those cans are stored on the shelf..  
  • Heated foods likes soups and stews when those foods are not properly cooled or are stored at the elevated temperatures (not refrigerated).
  • Vacuum packaged cooked foods that are not stored at the right temperature.
 Clostridium botulinum is a spore-forming organism, and these spores can survive normal cooking.  If conditions are right, then the spores germinate and the organism grows in the food.  Conditions include a notorious food, warm to high temperatures, non-acidic pH, and low oxygen level.

The toxin is a neurotoxin and stops vital functions including breathing.

Botulism Poisoning Cases Result from CA Company Processing Errors

A California company is recalling a number of jarred products after two people in Ohio were diagnosed with botulism poisoning.  Both individuals are improving after being on ventilation.  The FDA Notice (below) indicates the products may have been processing errors.

Many have predicted that with so many small entities entering the processed food arena, including through the farmers' market channel,  that it was only a matter of time before there would see processing deviations that would result in botulism cases.  Although FDA has strict regulations regarding canning foods that include requirements for registering of processes, training, testing, etc., many small entrepreneurs are either ignorant to these regulations, are incapable of implementing them, or choose not to follow.

You can walk through any farmers' market and see the vast array of jarred food items.  Are they acidified properly?  Have they received the proper thermal treatment?  Guarantee these are the same people fighting not to have to follow these 'restrictive government regulations'.  But trust us, there is a reason that that those regulations are in place.

Clostridium botulinum is a spore-forming pathogen that produces one of the strongest toxins known.  The organism can survive boiling temperatures and if the jarred product does not have any barriers (low pH, preservatives, strict refrigeration), the organism will grow and produce toxin.  The toxin, a neurotoxin, travels into the bloodstream to the nerves and it stops neurotransmission to muscles. And so when one ingests this toxin and does not receive quick medical attention, they will die of suffocation.  A hell of a way to buy the farm, so to speak.

No doubt, this farm/company will also have a difficult road ahead, from an intensive investigation to lawsuits.  That is a huge price to pay for not making the effort to produce safe products (unless the product is not to blame, and it was found to be consumer practices.)

In any event, hopefully this will be the 'shot across the bow' for state and federal officials to begin focusing attention on this developing cottage industry.

Botulism and Home Canned Elk Meat

Each year, those one or two cases of botulism that show up in the news serve as reminders of the importance of following proper processing (and using common sense). In summary, Mr. O’Connell thought he would short cut the canning processing by not going through the full heat process so that he could get more jars of elk meat. Rather, he just got the cans to form a vacuum seal and then moved on to the next batch. And when he heard one of the jars pop (lose vacuum) a week later, he threw it in the refrigerator. A few days later, he ate it for supper. Being a learned man, actually a lawyer, he knew something was wrong when his vision got blurry and his legs become wobbly. It progressed to a point where he lost all strength and had shallow breaths. His doctors were able to figure it out in time to save him.

He did use a pressure canner, but failed to follow process. Then instead of tossing the jar with the popped lid, he ate it.

Home Canning Hobby Leads to Near-Fatal Medical Emergency

Home canning is regaining popularity as part of the local food movement. If done right, families can enjoy home grown fruits, vegetables and even meat all through the winter. But if done wrong, it can be devastating, if not deadly.
A lawyer for the state of Washington recently learned that lesson the hard way.

On the Friday before Mother’s Day this year, Mike O’Connell was looking forward to spending the weekend with his wife at their home in the Seattle area. During the week, he lives alone in Olympia where he works. But he woke that morning with the strangest affliction: double vision.

“There were two of everything and I had an awful time just shaving and getting ready for work,” O’Connell said.



O’Connell, 67, is chief counsel to Washington’s Legislative Ethics Board. He suspected the double vision was related to some laser eye surgery he recently had. He managed to make it into work, but soon went home. That evening, he experienced more strange symptoms.

“My legs felt rubbery,” he said.

The next morning, he felt even worse. He was bumping into walls. He called his wife.

“I told her, ‘You know, I’m going to stop by the ER on the way up just so somebody can tell me I’m okay and I’m not having a stroke,”’ he said.

At the hospital, that’s exactly what they thought he was having. He heard “stroke in progress” called over the intercom. Suddenly he was surrounded by nurses and doctors. O’Connell’s wife arrived. Test results started coming back. There was no evidence of stroke.

“I didn’t know enough to bring up the fact that I had eaten canned meat,” said O’Connell.

Canned meat. You see, the night before O’Connell woke up with double vision, he had eaten some elk meat from a hunting trip. He canned it himself about a week earlier.

“Borrowed a pressure cooker, used an old family recipe for canning,” O’Connell said.

O’Connell’s mother had canned everything when he was a kid. He wanted to recapture a bit of his childhood. But things started going wrong from the start.

I had way too much meat to deal with,” said O’Connell.

The pressure cooker was too small. O’Connell had already browned the meat in a cast iron pan. So he decided to shortcut the process. Once the jars sealed airtight he would take them out of the pressure cooker and start a new batch. The next day, he heard a pop in the pantry.

“Which I remember as a child was the signal for you’ve lost the seal,” said O’Connell.

O’Connell found the jar with the popped seal, put it in the fridge and ate it the next day. He says it was delicious. The following week he heard another lid pop. Just as he had before, O’Connell found the jar and stuck it in the fridge. And a few days later he ate it for supper.

“This time, it didn’t work out,” O’Connell said.

O’Connell had an upset stomach in the night, but he didn’t connect it to having eaten the meat. He says growing up, he didn’t know anyone who got food poisoning from home canned foods.

At the hospital, once doctors ruled out a stroke, O’Connell was sent home. But he was back in the hospital a few hours later. Now he was having difficulty swallowing. The next morning, Mother’s Day, O’Connell’s daughter, Kelly Weisfield, drove to Olympia to see her dad.

“His voice was very slurred and his eyelids were droopy, but he was sitting up in bed and he was communicative,” Weisfield said.

As the day progressed though, O’Connell’s condition got markedly worse.

“By now, my eyes were closed. My strength—it was just amazing how quickly that went,” O’Connell said.

His breathing was getting shallow. Daughter Weisfield was frustrated with the lack of answers and scared. She called a doctor she knew, a neurosurgeon. He ran through a short checklist of things to rule out. That list included a disease first identified in the 18th century: botulism. Weisfield looked it up online.

“It just made the hair on the back of my neck stand up because it was every single symptom just laid out exactly what my dad was experiencing,” she said.

Botulism is a paralyzing illness caused by what Centers for Disease Control calls the most potent toxin known to science. It’s rare; there were only 20 foodborne cases nationwide in 2011, just one in Washington state last year.

Improperly home canned foods are the leading culprit, especially those low in acid like green beans and, yes, meats. Weisfield called her mom who had just left the hospital.

“And I said, ‘Mom, turn around. You got to go back and tell them to look into this,’” Weisfield said.

Weisfield was relieved, but also terrified that it was too late. Her father could hardly move now. He was having more and more difficulty breathing. The hospital had parked a ventilator outside his room. Weisfield didn’t know what to tell her 10-year-old son, who is very close to his grandfather.

“First thing Connor said was, ‘Are we still going to go on our fishing trip?’ And I could never answer him, because I didn’t know,” she said.

The doctors didn’t even wait to confirm botulism. They ordered a dose of anti-toxin from the CDC. Now the medical mystery was solved. But how did O’Connell get botulism?

Remember he stopped cooking the jars of elk meat when he heard the seals lock in place. Washington State University food safety expert Zena Edwards says that was O’Connell’s nearly fatal mistake.

“All that indicated was it had now become an anaerobic environment, an oxygen-free environment,” Edwards said.

And that’s the strange thing about the bacteria that causes botulism. It thrives when deprived of oxygen. By shortcutting the cooking time, O’Connell failed to kill the bacteria. Instead, he sealed it into the perfect environment for it to produce the poisonous toxin.

Edwards says what happened to O’Connell reaffirms two cardinal rules of home canning: “plan before you can” and “when it doubt, throw it out.”
After receiving the anti-toxin, O’Connell transferred to Swedish Hospital in Seattle for rehab. It took just days for the Botulism to paralyze O’Connell. The recovery would be painfully slow.

“My eyes were the first thing to come back. I still walk with difficulty and use a cane. I have no taste with the exception of chocolate, so I buy chocolate ensure, chocolate mints and night before last, I found where they sell chocolate wine so I had some of that, too,” O’Connell said.

O’Connell doesn’t know if or when he’ll get his taste back. Before the botulism, he was fit and active—a hunter and avid hiker. His daughter says it’s hard to see her dad like this.

“I’m so grateful that he’s made it through. And I’m so sad that he’s gone through all this, and he’s not the same,” Weisfield said.

O’Connell was able to keep that promise he’d made months ago to his grandson to go fishing together on the upper Columbia River. As for future home canning projects, his family has made it clear that’s not going to happen.

2 cases of botulism linked to tofu

Tofu, sold in NY City, has been linked to 2 cases of botulism. The tofu was purchased by a couple in a NY City store. The tofu was displayed in a non-refrigerated, water filled bin. This is the first cases of botulism in NYC in more than 15 years.

Tofu is made by coagulating soy milk and pressing the resulting curds. The process begins by soaking, grinding, boiling and straining dried or fresh soybeans. Coagulation of the protein and oil (emulsion) suspended in the boiled soy milk is the most important step in the production of tofu. This process is accomplished with the aid of coagulants. Two types of coagulants (salts and acids) are used commercially. The third type of coagulant, enzymes, is not yet used commercially but shows potential for producing both firm and "silken" tofu’. Clostridium botulinum spores would survive the boiling and then be able to grow in the non-refrigerated product (in the absence of a secondary inhibitor. Tofu products vary greatly, even in terms of inclusion of antimicrobial parameters.) The spores will germinate and grow in the protein rich, non-refrigerated product. The product sitting in water would ensure a sufficient water activity as well as help support an anaerobic environment, both essential for C. botulinum. As C. botulinum grows, it produces a neurotoxin. If ingested, the neurotoxin enters the bloodstream and shuts down muscle function, potentially leading to death through the inability of the victim to breath.

Tofu is a considered a potentially hazardous food (TCS food) and thus requires refrigeration during distribution, at retail, and by the consumer.
