Improper Line Clean-out in Snack Process - Whole Foods Market is voluntarily recalling 365 Everyday Value White Corn Tortilla Chips because the product may contain undeclared milk that was not listed on the product label.  The issue was discovered after a customer notified Whole Foods Market that the tortilla chip bag also contained other snack mix products.

Improper Line Clean-Out in Dairy Process - Harris Teeter is notifying shoppers of a voluntarily recall of Harris Teeter Low Fat Frozen Yogurt Cookies & Cream due to undeclared peanut product.  The firm was notified of the problem via a consumer comment of finding peanut butter cups in the product. Subsequent investigation indicated that the problem was caused by a mix-up of the cartons.

This Week in Mislabeled Products for Week Ending May 18, 2019

Salad with Chicken with Undeclared Soy - Caito Foods LLC., an Indianapolis, Ind. establishment, is recalling approximately 1,767 pounds of salad with chicken products due to misbranding and undeclared allergens, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The products contain soy, a known allergen, which is not declared on the product label.

NY State Finds Product Label Missing Sulfites - US TRADING COMPANY is warning consumers not to consume GLOBAL PRIDE RATTAN SHOOT IN BRINE. The product is being recalled because the product contains Sulfites which is not declared on the labeling. The recall was initiated after routine sampling by New York Department of Agriculture and Markets Food Inspectors. After analysis, 555.0 ppm of sulfite was detected in the Global Pride Rattan Shoot in Brine 24 ounce glass jar which were not declared on the label.

Milk in Non-Dairy Frozen Dessert - Coolhaus is voluntarily recalling its Dairy Free Horchata Frozen Dessert Sandwich because it may contain milk.  Only one consumer complaint has been reported to date.

The Mayonnaise Has Soy GHSW, LLC is voluntarily recalling select salads, wraps and salad bar trays sold two Whole Foods Markets 365 in Texas and at Whole Foods Markets in Louisiana, Oklahoma and Texas because the product labels fail to declare soy as a sub-ingredient of the mayonnaise used to manufacture the products.  Some of the labels, however state that the products "may contain soy."   The recall was initiated after it was discovered during a label review that mayonnaise containing soy was used in these products and that the packaging did not properly declare the presence of soy.

Missed the Anchovies - MIBO Fresh Foods LLC, a Fort Worth, Texas establishment, is recalling approximately 1,460 pounds of salad with meat products due to misbranding and undeclared allergens, the product contains anchovies which was not declared on the label.  The problem was discovered by a retail store when the mislabeled products were being unboxed to be placed on the retail store shelves.

Missed the Anchovies - Taylor Farms Texas Inc., a Dallas, Texas establishment, is recalling approximately 1,079 pounds of Caesar salad with chicken products due to misbranding and undeclared allergens, the product contains anchovies (fish allergen) which is not declared on the product label. The problem was discovered by a retail store when the mislabeled products were being unpackaged to be placed on the retail store shelves.

Whole Foods Takes Warning Letter on the Chin for February Inspection

FDA issued a warning letter on June 8, 2016 to Whole Foods for an inspection completed back in February.  While Whole Foods responded to the inspection findings in March, the Warning was issued this week (June 8th), because FDA did not find the response acceptable.
"We do not consider your response acceptable because you failed to provide documentation for our review, which demonstrates that all your noted corrective actions have been effectively implemented. This documentation may include photographs, invoices, work orders, voluntary destruction records of any affected products, certification of actions performed by contractors, and/or any other useful information that would assist us in evaluating your corrections."

The initial inspection found a number of issues, nothing overly serious. but mostly what could be considered sloppy procedures.  This included multiple condensation issues, improper sanitation procedures with instances where too high a sanitizer concentration was used, potential cross contamination issues, inadequate handwashing water temperatures, and improperly marked sanitizing agents.  Environmental sampling of 100 samples was negative for Listeria monocytogenes, but one sample was positive for a non-pathogenic species of Listeria.

Clearly some bad vibes going on between the agency and the plant.  If issues were so bad, why did a follow-up inspection not occur, or why wasn't this letter issued in March?  A lot to take away from this.

  • Tighten up procedures now rather than having an agency inspector tell you.  Inspections are rarely perfect, but the number of issues seen here show a lack of attention to detail.
  • Ensure corrective actions to inspection reports are completed as soon as possible.Those corrective actions must be detailed / documented to cover each and every element addressed in the inspection and be able to demonstrate that the risk issues have been adequately  addressed .  Overkill in documentation support was needed here rather than glossing over.

Illinois Firm Recalls Veggie Pizza Due to Undeclared Milk

Richelieu Foods Inc. Wheeling Illinois has voluntarily recalled the 365 Whole Foods Market Vegan Ultimate Veggie Thin Crust Pizza, LOT CODE 06152024 with a best by date of 6/15/2024 because it may contain trace amounts of an undeclared Milk allergen.  This recall involves no more than 144 individual pizzas that inadvertently entered the supply chain. The product in question was distributed in some retail stores in the states of Louisiana, Texas, Arkansas, and Oklahoma.

Almost sounds like product-on-hold that somehow got shipped?

This Week in Mislabeled Products for Week Ending 8-21-2020

Ingredient Missed on Label - Whole Foods Market is voluntarily recalling Vegan Vanilla Cake from 21 stores across Alabama, Georgia, Mississippi, North Carolina, South Carolina, Tennessee and one store in Tallahassee, Florida, due to an undeclared soy (soy milk) allergen.  This is a print-and-apply label.....seems like somebody needs a better process for inputting and verifying ingredient information on this print-on-demand labels.  

Brazil Nuts in Mixed Nuts, but Don't Belong There - Superior Nut Company of Cambridge, MA is recalling Southern Grove Mixed Nuts Less than 50% Peanuts, because it may contain undeclared Brazil Nuts. The recall was initiated after it was discovered that product containing Brazil Nuts was distributed in packaging that did not reveal the presence of Brazil Nuts.

Wrong Potato Chips in the Bag - Frito-Lay announced a limited voluntary recall of a very small number of $1.29 1 1/2 oz. bags of Ruffles Original Potato Chips because they may contain undeclared milk ingredients.  The recall was initiated after it was discovered that a small number of bags of Ruffles Original Potato Chips were inadvertently filled with another flavor of potato chips, potentially exposing consumers to undeclared milk.

Soup with Pasta and Meatballs Labeled as Chicken Noodle - Faribault Foods Inc., a Faribault, Minn. establishment, is recalling approximately 15,134 pounds of canned soup product due to misbranding and undeclared allergen- the product contains milk and soy, known allergens, as well as beef and pork, which are not declared on the product label.  The cans labeled as chicken noodle soup actually contain a meatball and pasta product.  The problem was discovered when the firm’s distributor notified FSIS of consumer complaints that the soup contained meatballs and pasta instead of chicken and noodles.

VT Cheese Company Recalls Cheese Due to Listeria

Consider Bardwell Farm, Whole Foods Market stores in the Northeast and North Atlantic regions are voluntarily recalling Dorset cheese because of a potential contamination of Listeria monocytogenes.

According to the company's own recall notice, the recall was conducted for a number of product types and was done after Listeria was found on product as well as in the processing environment.  No illnesses have been reported.

On the company website, they state that all production has been halted.  Consider Bardwell Farm is located in VT.

Cheddar Cheese Recalled After Listeria Positive Test

Cahill's a cheese manufacturer located in Ireland is voluntarily a number of cheese products after internal testing found the presence of Listeria.  "Routine testing picked up positives of Listeria Monocytogenes. Our investigation identified a single piece of equipment in our facility as the potential source of this issue and the equipment was immediately removed from our production line. All our products manufactured since have tested fully clear."

Whole Foods is recalling Cahill Cheddar Porter and Whiskey Cheddar Cheeses from 44 stores across 22 states because of a potential contamination of Listeria monocytogenes. The issue was discovered when Whole Foods Market was notified by a Cahills of the positive test result for Listeria monocytogenes. There have been no know illnesses.

French cheese linked to a case of listeriosis

An elderly Pennsylvania man was stricken with listerosis after eating a high-priced soft cheese imported from France.  Whole Foods is recalling the cheese baring thier own label.

Interesting is the fact that Whole Foods looks to have cut and repackaged the cheese which begs the question...where did the contamination occur - was it present in the original package, or did the cheese become contaminated during repackaging?  Additionally, the article stated that the cheese product was slow moving in regard to retail sales, which means that it would have given more time for Listeria to grow.  Listeria, as we know, grows at refrigeration temperatures, although slowly.

A pricey French cheese sold at Whole Foods Market in East Liberty is being recalled more than a month after a 69-year-old Westmoreland County man grew seriously ill from listeriosis, a food-borne bacterial infection.

The man fell ill June 7 and was hospitalized after eating Jean Perrin Edel de Cleron cheese -- a soft, pasteurized cow's milk French cheese that sells for about $25 a pound. The man continues to recover in a rehabilitation center, said Guillermo Cole, Allegheny County Health Department spokesman.

On Thursday, the health department and Whole Foods Market announced the recall of the cheese, which was cut and packaged in clear plastic wrap bearing a Whole Foods Market scale label and code beginning with 293351. The cheese was sold between May 20 and July 3.

The recall didn't occur until more than a month after the man fell ill, Mr. Cole said, because it took time for him to be diagnosed and then to get positive test results on foods he had consumed with follow-up efforts to test samples from the market where he purchased the cheese.

"It required some medical detective work," Mr. Cole said.

The store is offering full refunds for the cheese. The health department said customers who purchased it from the market during that time period should either dispose of it in the garbage or return it to the store. Those who touch the cheese should wash their hands immediately afterward to avoid cross-contamination. The store also has posted signs in the store to notify customers of the recall.

The store and health department are working to ensure that no cross-contamination occurred and that equipment and utensils used in cutting, weighing or packaging the cheese isn't contaminated with listeria monocytogenes, the bacterium that causes listeriosis.

The illness primarily affects older adults, people with weakened immune systems, pregnant women and newborns. Such infections, caused by consumption of food contaminated with the bacterium, can be serious and possibly fatal.

Anyone who consumed the cheese should seek medical help if the individual has diarrhea and gastrointestinal illness followed by fever and muscular aches.

Customers who have the recalled product may contact the health department at 412-687-2243 (ACHD) and arrange to have the cheese tested.

The health department declined to identify the ill man. Mr. Cole said the man represents the only one affected by the contamination to date. The cheese that the man purchased tested positive for the bacterium, as did samples of the cheese taken at the store.

On the upside, Mr. Cole said, the price of the cheese meant that not a large quantity had been sold.

This Week in Mislabeled Product - Week Ending 6/30/2018

Milk Not Added to Label - A Boston, Mass. establishment, is recalling approximately three pounds of chicken wrap products due to misbranding and undeclared allergens - the products contain milk which is not on the label.  The problem was discovered by FSIS personnel during routine in-plant verification activities. (Label design error).

Supplier Change in Formation Not Communicated - Whole Foods Market is voluntarily recalling Whole Foods Market brand oatmeal raisin cookies from stores in eight states because the products contained soy flour that was not listed on the product label. The problem was discovered when discovered when the supplier notified the company of an unannounced ingredient change. (Supplier error).

Mislabeled Salad Dressing - Pinnacle Foods Inc. is voluntarily recalling a limited quantity of Wish Bone House Italian Salad Dressing  after learning from a retail customer that a limited number of bottles were mislabeled. The product contains milk and egg, known allergens not declared on the bottle. (Production error in labeling).

Macadamia Nuts Recalled Due To Salmonella Positive Sample

Whole Foods is recalling raw macadamia nuts after an FDA contract laboratory found a sample of the product to be positive for Salmonella.

Macadamia nuts are often eaten raw.  The processing is fairly simple - Dehusking, drying, de-shelling, and further drying to a moisture level of 1.5%.  Contamination can occur from the point when the nut is de-shelled until when it is packaged.

There have been a number of recalls of macadamia nuts due to Salmonella (Nov 1, 2014, 2010 and 2009).

Recall of Salad Dressing Due to Undeclared Allergen Recall Expanded

VanLaw Food Products Inc. is expanding its recall (in late August) of Whole Foods Market 365 Organic Creamy Caesar Dressing because it contains undeclared Soy and Wheat allergens. The recall is being expanded to include Whole Foods Market 365 Organic Creamy Caesar Dressing with a BEST IF USED BY DATE of SEP 21 22 through JUN 06 23 and incorrect UPC Code 99482-49027.  The recalls were initiated when it was discovered the products were distributed in packaging that did not reveal the presence of Soy and Wheat allergen. The problem was caused when the back label from another product was mistakenly put on the products being recalled.

Orangegate - Packaging of Pre-Peeled Oranges

Whole Foods took some criticism for selling pre-peeled oranges in plastic packaging.  A person's tweet went viral stating “If only nature would find a way to cover these oranges so we didn’t need to waste so much plastic on them.”  Whole Foods quickly discontinued sale.

Not only does it create waste, but removing one of the best food safety packages around, the orange peel, the product becomes more susceptible to contamination.

Two Retailers Issue Recalls Associated with Mann Packing Recall of Produce Due to Listeria

Whole Foods and Albertsons have issued recalls associated with vegetable items from Mann Packing Company due to Listeria.  Mann Packing issued their recall on November 3rd.

The Listeria issue evidently started with testing done by the CFIA.  From an article in The Packer,  "According to the Canadian Food Inspection Agency, the Mann Packing recall was an update to a Nov. 1 recall of six Compliments (Sobeys) brand fresh-cut products. That recall was also an expansion of an Oct. 30 recall, of Compliments Sweet Kale Blend. The kale item was recalled following a test from the Canadian agency that detected Listeria monocytogenes."

Grocery Chain Recalls Deli Salad Items Due to Listeria Positive Samples

Whole Foods recalled various deli salads including chicken and pasta salad made at their central kitchen in Massachusetts.  Product was made on the 16th with a sell-by-date of the 23rd.
At the time of the recall, the product had already reached its expiration date.

A few things to note - the notice states that a sampling of the products tested positive which indicates more than one product.  Whether or not that is the case, they do have Listeria within the facility and there should be a question of whether it is under control.  Commissary operations may or may not do extensive testing for Listeria, mainly because the shelf-life is so short.  Since this product is one day beyond the Food Code limitation of 7 days (day one is the day product is made), there should be controls in place.  Also, since this is a dual jurisdiction plant (both FDA and USDA have oversight), from a USDA perspective of a facility with RTE product and post-lethality exposure, some testing should be taking place.

When Listeria is found on product, there is a high probability that it will still be in the environment unless there is a good explanation why contamination was limited to that day.  Hopefully before this facility restarts, they conduct ample cleaning and verification testing prior to starting production.

No illnesses have been reported...yet.  With Whole Genome Sequencing, further analysis of the LM found in the facility can now be matched against cases of Listeria infection that have occurred throughout the region.

Supplier Packaging Error - Giant Eagle, Inc. has issued a voluntary recall of Giant Eagle Dried Island Fruit Mix due to the possibility the product may contain undeclared Peanut, Almond, Milk and Soy allergens.  Giant Eagle was made aware of the issue by a guest who purchased the product and the company has since learned that the cause was a packaging error made by the product manufacturer.

Wrong Back Label - Winter Gardens Quality Foods, Inc., is voluntary recalling one of its products: Whole Foods Market Vodka Sauce carrying the lot code 20057, because of an undeclared allergen (Milk).   The recall was initiated following a customer complaint regarding the incorrect back label which contained the wrong ingredient and allergen statement.

Sulfites Not Declared - Tiffany Food Corp. of Brooklyn, NY is recalling Huangfushanzen Huang Mountain Tea Mushroom (黄府山珍茶花菇) Dried Mushrooms with UPC 6 943153 804047 and no other coding, because it may contain undeclared sulfites. The recall was initiated after routine sampling by Florida Department of Agriculture and Consumer Services (FDACS) and subsequent analysis revealed the presence of sulfites in Huangfushanzen Huang Mountain Tea Mushroom (黄府山珍茶花菇) Dried Mushrooms which were not declared on the label at a level that could cause a life-threatening reaction in sulfite-sensitive individuals.

Pecorino Cheese Wrapped in Walnut Leaves Recalled for Listeria

Whole Foods and Forever Foods are recalling Pecorino Aged Cheese wrapped in Walnut Leaves after Forever Foods completed testing and found the sample positive for Listeria.
