Expansion of Fruit Puree Pouches Due to Elevated Lead Levels, Now 22 Reported Cases

FDA provided an investigation update on November 13, 2023 where they stated "As of November 13, 2023, there have been 22 reports of illness potentially linked to recalled product submitted to FDA."

CDC reports "As of November 7, 2023, there are 22 cases, in states including Alabama, Arkansas, Louisiana, Maryland, Missouri, New Mexico, New York, North Carolina, Ohio, Pennsylvania, South Carolina, Tennessee, Texas, and Washington, ages 1 to 3 years, with BLLs ranging from 4 to 29 micrograms per deciliter (µg/dL). Cases experienced signs and symptoms including headache, nausea, vomiting, diarrhea, change in activity level, and anemia."
While any levels of lead in the blood is bad,  CDC uses a blood lead reference value (BLRV) of 3.5 µg/dL to identify children with BLLs that are higher than most children’s levels.

Snake Pizza - Considerations for the Hazard Analysis

An news article on CNN discusses how Pizza Hut is teaming up with a century-old Hong Kong restaurant to put a modern spin on a traditional dishes, in this case, snake on a pizza.  "The new offering combines shredded snake meat, black mushrooms and Chinese dried ham – all indispensable ingredients of an authentic snake stew and part of the Hong Kong franchise’s marketing plot to generate buzz online."  "The company is also introducing a pizza made with Chinese preserved sausages featured in a claypot rice dish".

"It is not the first time Asian franchises of the original American brand have adopted a more adventurous gastronomical approach to celebrate local food culture.  In the past, Pizza Hut in Taiwan introduced pizzas made with durian, pig blood curds, and preserved eggs – ingredients that play a big role in the local culinary culture.  Meanwhile, Pizza Hut in Japan created a pizza topped with Tonkotsu ramen (pork bone soup ramen)."

As food companies look to integrate local flavors to build excitement in those markets, they will use ingredients where there may be less knowledge regarding hazards.  This can be a big challenge to food safety experts - determining risks of unique ingredients where there may be limited formal study.  Then, making sure sufficient controls are in place for hazards.

Hong Kong's Pizza Hut is partnering up with a local century-old restaurant to introduce a new snake-soup pizza.

Investigation of 2020 Salmonella Outbreak in Restaurant Finds Drain Contamination as Likely Source for Ongoing Issues

An investigative report in the Journal of Food Protection provides the approach and findings related to a 2020 Salmonella outbreak which "identified a total of 67 cases between January 1, 2020, and August 13, 2021, 66% of which were directly linked to a restaurant in the area".   Regulatory officials conducted a intensive investigation, looking at many angles, but in the end, they believe the endemic contamination was linked to the kitchen drainage system.
"Explanatory hypothesis adopted. The most plausible explanation for the origin of this outbreak remains the contamination of the drains in Restaurant A. Indeed, most of the environmental samples positive for Salmonella were found in sinks or near them (sink sprayers, drains). The presence of contaminated biofilm (Mair-Jenkins et al., 2017) in the restaurant’s kitchen drainage system may have had a role to play in the extended duration of this outbreak."

A few takeaways:

  • The importance of periodic drain cleaning. This occurs in food operations, but not really discussed to any degree for foodservice operations. Depending on the location of the drain relative to operational areas and the potential for the drains backing up, the risk of contamination from drains can vary. However, a periodic cleaning regiment can reduce this risk.
  • During investigations,  much focus is placed on a given food item.  In this case, an environmental niche may have been the source to many food items via contaminated equipment or hands.  These out-of-site niche areas can foster resident or endemic contamination that can be difficult to identify if only looking at given food items.

Animal Feed Premix Facility Issued Warning Letter for Improper Control of Vitamin D

FDA issued a Warning Letter to ADM Animal Nutrition and their  vitamin and mineral premix and preblend animal food manufacturing facility, located in Effingham, Illinois.  The inspection was done in response to the premix with high levels of Vitamin D.  Clearly a significant hazard, the chemical blending of Vitamin D was controlled through a basic prerequisite program rather than as a Preventive Control.
"Specifically, your firm manufactures a preblend that is subsequently used by other firms in the manufacture of animal food products, and your reliance on your prerequisite programs failed to reduce the probability that a vitamin D nutrient toxicity will occur in your animal food in the absence of a preventive control."

FDA Releases Pesticide Residue Monitoring Report 2021

The U.S. Food and Drug Administration (FDA) has released the report for its fiscal year (FY) 2021 pesticide residue monitoring program, summarizing findings from FDA testing of human and animal foods for approximately 750 different pesticides and selected industrial compounds between October 1, 2020 and September 30, 2021.
  • Overall, 1,367 total human food samples were collected, including 300 domestic food samples from 26 states and 1,067 imported food samples from 66 countries/economies. Of the samples collected, 96.7 percent of domestic samples and 89.3 percent of imported samples were compliant with federal regulations (below EPA tolerances). No pesticide chemical residues were detected in 35 percent of domestic samples and 44.5 percent of imported samples.
  • A total of 80 animal food samples were also collected, including 16 domestic food samples from five states and 64 imported samples from seven countries. All of the domestic samples and 98.4 percent of imported samples were below EPA tolerances. No pesticide chemical residues were detected in 37.5 percent of domestic samples and 40.6 percent of imported samples.
From the report:
Imported Products That May Warrant Special Attention
The design of the FDA pesticide program focuses on products that have a history of  violations or are suspected of violations, based on information such as reports from other agencies and pesticide usage data. Historically, the violation rate for import foods is higher than for domestic foods; results from FY 2021 continue that trend.
The violation rate for  import foods (10.7%) was over 3 times higher than the rate for domestic foods (3.3%). The majority of the violations for import commodities are no-tolerance violations, with
approximately 82% of the violative residues < 0.1 ppm.

TX Firm Recalls Dog Food After Testing Finds Salmonella

Mid America Pet Food, Mount Pleasant, Texas, is voluntarily recalling three lots of Victor Super Premium Dog Food, Select Beef Meal & Brown Rice Formula produced at its Mount Pleasant, Texas production facility because it has the potential to be contaminated with Salmonella. This voluntary recall was initiated after a third-party conducted random sampling and product associated with three lots tested positive for Salmonella.

Contamination of dry extruded pet food by Salmonella most often occurs as post-process contamination.  This contamination can come through contaminated ingredients added to the product after extrusion, or through environmental contamination occurring after extrusion, somewhere from drying through bagging.  Contamination events, in these cases, often affect a wider range of production lots.  So depending upon the investigational findings, it may be expected to see an expansion of this recall.

Raw Pet Food Recalled After Salmonella Detected in Product

Blue Ridge Beef of Statesville, NC, is recalling their Breeders Choice 2# log dog food Lot# 8/2/24 UPC 8 54298 00193 1 due to Salmonella contamination. On October 27, 2023, the firm was notified by the Food & Drug Administration that the dog food test positive for Salmonella.  The website mentions HPP, so it may be assumed that HPP (high pressure processing) is used a pathogen reduction step.

The company, affiliated with Lea-Way Farms, was issued a Warning Letter in June of 2020 by FDA.
An article in Pet Food Processing (2020) summarizes this report,  "The FDA reported violations to Current Good Manufacturing Practice (CGMP) requirements, including failure to properly inspect and handle raw materials, as well as unsanitary conditions in several areas of the plant.
Specifically, the FDA cited the use of raw animal ingredients “…that have died otherwise than by slaughter… without first determining whether the animals suffered any type of illness, injury, and/or whether any medications may have been administered to the animals prior to your pick up from the supplier…” The agency also observed the potential for contamination on the facility’s kill floor, grinder/mixing room, in the cooler, and in its overhead conveying rails between the kill floor, cooler room and processing area."

Within the 2020 FDA investigation report, it states: "During our inspection, FDA collected final product and raw ingredient samples for microbiological samples. Sample #1098421 consisted of an in-process sample of raw ground beef intended to be used as an ingredient in your firm’s finished product. This sample was found to be positive for Salmonella London Group B1 and generic E. coli. Sample #1098422 of a finished product sample of Blue Ridge Beef Raw Kitten Grind was found positive for Salmonella Agona Group B and Listeria"

TX Pet Food Processing Facility Recalls Single Lot of Dog Food Due to Salmonella Positive Sample

TFP Nutrition of Nacogdoches, TX announced today that it is issuing a voluntary recall of Retriever All Life Stages Mini Chunk Chicken Recipe Dry Dog Food due to potential Salmonella contamination.  This voluntary recall is being issued due to a single sample of Retriever All Life Stages Mini Chunk Chicken Recipe Dry Dog Food that tested positive for Salmonella in a random sample test conducted by the Office of the Texas State Chemist.

This recall is currently for a single lot.  This is a case where depending on the investigation, the recall can be expanded if it is found that the strain of Salmonella is endemic, or established, within the facility, in which case, more than one lot would be impacted.  Salmonella is a significant environmental hazard in the post-process environment.  Salmonella can survive for an extended time in this dry environment, posing a contamination threat to exposed product prior to packaging.

GA Produce Company Recalls Bagged Collard Greens After State Lab Detects Listeria in Sample

Baker Farms of Norman Park, GA is voluntarily recalling a single production run of Kroger 16-ounce bagged Collard Greens, due to possible Listeria monocytogenes contamination.  The issue was discovered by the Tennessee Department of Agriculture while conducting routine product testing.  To date, there have been no reports of illness related to the consumption of this bagged collard green product.

As we have seen in past Listeria-related recalls, there is always potential for an expansion from the single lot.  Much of this will come as the investigation determines the degree to which Listeria control was insufficient, allowing an established a strain of Listeria to become established in the facility.  In this case, it is likely that more than the one lot was affected.

Here is a look at the facility...about 1 minute into the video.

Candies Containing Konjac Gum Recalled Due to Potential Choking Hazard

Tiffany Food Corp of Brooklyn, NY is recalling all codes and flavors of Mini Fruit Jelly Cups, because they may pose a choking hazard. Small jelly cups containing konjac powder have previously been implicated in choking deaths of children. Konjac and its consequent texture and consistency could pose a choking hazard to small children as well as adults with functional and/or anatomic abnormalities predisposing them to dysphagia. The potential choking hazard was confirmed after discussion with a representative from the Food and Drug Administration.

Food Additives Council
https://www.foodingredientfacts.org/facts-on-food-ingredients/sources-of-food-ingredients/konjac-gum/
  • Konjac gum, or konjac flour, is a hydrocolloid (water soluble) dietary fiber derived from the Amorphophallus konjac plant (also known as devil’s tongue) that is used to provide texture, thicken, stabilize, emulsify, and suspend ingredients in foods and beverages. Konjac gum is primarily used as a source of glucomannan, which is a recognized source of dietary fiber.
  • Konjac gum is safe for children in moderation and often used in medicines. However, certain forms of konjac gum have been recalled in certain countries because of the risk of suffocation from consuming large amounts due to its thickening properties. The gum runs a risk of expanding in your throat if you consume too much of it at once. This is why manufacturers adhere to regulations on the amount allowed in foods to ensure product safety.

Updated Preventive Controls Training Highlights HACCP-link and Added Benefits Of Additional Controls

An article in the month's Food Safety Magazine titled FSPCA Preventive Controls Updated Curriculum Highlights—Advantages for Food Safety Management discusses the updated version of FSPCA's training curriculum. Version 2.0 marks a few noticeable changes in addition to updating content to address FDA guidance releases. One of those changes is a rearrangement of material to have additional emphasis placed on Process Preventive Controls or CCPs. This aligns better with traditional HACCP training. Another change was the use of additional examples to demonstrate when aspects of programs for Allergens, Supplier Control, and Sanitation become Preventive Controls.

The article also discusses the advantage of the Preventive Controls approach over HACCP. Using a more intensive control approach to aspects of sanitation, allergen control, and supplier control can go a long way in reducing some of the primary causes of recalls currently being issued by companies. This is certainly the case for allergens, where allergen related recalls due to lack of adequate control are too common of an occurrence.

The updated curriculum is expected in early 2024. If you have taken training using version 1.2 and still have questions regarding the development or implementation of you plan, you may want to consider attending this updated training. To make this more feasible, Penn State will offer the updated course to those who had previously taken the course through Penn State at a discounted price. More information to come on that.
